That's a rather silly question... It's no different in OpenSim than in SL...

The Cool VL Viewer however got a genuine v1 inventory UI, meaning an "outfit" is any folder you want to store an outfit into, anywhere in your inventory (but for some special folders like trash, etc, of course). Note that for this v1 viewer, the v2+ viewers "My Outfits" folder got no special meaning (you may even delete it if, like me, you store your outfits in other folders), but you may nonetheless use it if you wish.

For creating new outfits, it however got a specific floater (you may open it via the "Inventory" floater "File" menu, "Make new outfit" entry, or via the "Appearance" editor floater, "Make outfit..." button) that allows to choose which currently worn body parts, wearable layers and attachments you want to retain for your new outfit (do read the tool-tips for the various options in that floater: they are worth a full documentation). Note that you may change parts, add or remove layers or attachments while the floater is open: it will auto-refresh accordingly.

You may also configure in which folder the new outfits sub-folders will be created (it's "Clothing" by default), by right-clicking on that folder in your inventory and choosing "Use as default for >" --> "New outfits".

To change an existing outfit, simply add or remove items to/from the corresponding outfit folder.

If you wear several layers of the same wearable type and changed your outfit, you may also refresh the layer numbers for all linked worn items with a right click on the inventory folder of your worn outfit and choosing "Update layers info" from the context menu (just like in other viewers in fact).
